Members Included in Each No Copay Medical Care Membership Plan
- Individual No Copay Medical Care Membership Plan: Membership for 1 person
- Individual + One No Copay Medical Care Membership Plan: Membership for 1 person and spouse or child. Child must be age 0-25. Members must reside in the same household.
- Family No Copay Medical Care Membership Plan: Membership for 1 person, spouse, and up to 6 children. Children must be ages 0-25. Members must reside in the same household.
Activation Date for your No Copay Medical Care Membership Plan
The activation date option determines when you want your No Copay Medical Care Membership Plan to become active. Your choices are one of the following:
- 1st of next month for $0.00
- Within 3 business days for $69.00
All No Copay Medical Care Membership Plans include
- Unlimited Primary care visits
- Unlimited Urgent care visits
- Mental health visits (12/year per member)
- Wellness visits-including well child immunizations
- Blood draws and lab work
- X-rays for acute injuries
- Weight loss solutions
- Access to all participating clinics
- No copays
- No Deductibles
